While a fever is often the body’s way of fighting off an infection, it’s important to watch for other symptoms that may point to something more serious. In most cases, fevers go away within a few days — but when certain signs appear alongside a high temperature, it may be time to call your doctor.

5 Symptoms to Watch For

Here are five warning signs that, when combined with a fever, may indicate a more serious illness. If your child shows any of these symptoms, especially with a high or persistent fever, contact your healthcare provider right away.

1. Lethargy or Unusual Fatigue

If your child seems unusually tired, isn’t eating, appears pale or flushed, or shows other changes in energy or behavior, this could be a sign that their body is working hard to fight off something more serious.

2. Rash That Doesn’t Fade

Skin changes can offer clues about what’s going on internally. Be especially cautious if your child develops small purple-red spots that don’t fade when pressed, or large purplish blotches.

3. Trouble Swallowing

If your child is drooling excessively and having difficulty swallowing, this may be a sign of throat inflammation or another issue that needs medical attention.

4. Breathing Difficulty

If your child struggles to breathe — even after you’ve cleared their nasal passages — seek medical care. Labored breathing, wheezing, or rapid breaths should never be ignored.

5. Mood and Alertness Changes

If your child seems disoriented, glassy-eyed, unusually cranky, or overly irritable, it could be more than just a fever. Trust your instincts when behavior feels “off.”

When to Call a Doctor

You don’t need to wait for a high temperature to reach out. If you feel unsure about your child’s condition, appearance, or behavior — even with a mild fever — it’s always best to check in with your doctor or pediatrician.

In addition to fever, be sure to mention other symptoms like:

  • Persistent cough

  • Vomiting or diarrhea

  • Suspected ear pain or ear tugging
